What is inside:

Two PDF files are included, one laid out for US Letter and one for A4. Both contain the same pages.

Supplied as a ZIP file containing the US Letter PDF, the A4 PDF and a READ-ME text file. Extract the ZIP before opening the PDFs.

This is a digital download. No physical item is shipped. The pages are static printables designed to be completed by hand — they are not fillable forms and have no built-in fields for typing. Some PDF apps offer separate annotation tools, but those features are not part of this product.
